Please respond to all of the application questions. You must adhere to all requirements as specified under Entry Requirements. Each application must specify the Award and your Utility Class (organization size of your utility) in which it will be judged.

To be considered for the award, your application form and any supporting documentation must be submitted, preferably in electronic format by February 17, 2012.

If any supporting documentation is in hard copy only, please send five (5) copies along with a copy of your application to the address below.
